2.Alert Reports – If the member is experiencing exacerbation of symptoms or biometric data,Alere will fax an Alert Report to the treating physician within a few hours of reported data.Alert Reports are issued at any time actionable information requires review by the physician within 24 hours.Additionally,Alere personnel will call the physician’s office to verbally relay the situation causing the alert; particularly if the member is exceeding physician-specified reporting parameters,is not compliant,or otherwise appears to be declining rapidly and requiring immediate intervention.Alerts may include,but are not limited to,weight graphs,blood glucose graphs,nursing assessment,and self-reported symptoms.With the use of Alere’s Alert Reports,the treating physician is able to modify the treatment plan and often prevent unnecessary hospitalizations for exacerbation of symptoms.
3.Status Reports – Alere also sends Status Reports to the treating physician.Status Reports are similar to Alerts reports,but are issued whenever there is new information that is pertinent and should be immediately transmitted but does not require physician review within 24 hours.
4.Pre-Visit Reports – Some of our programs provide a Pre- Visit Report to the physician One to two days before the member’s scheduled office visit,Alere faxes a Pre –Visit Report to the physician’s office that include,but not limited to,self-reported health numbers,blood glucose graphs,current self- reported medications validated with pharmacy benefit manager data,current and previous cardiac risk scores,A1C results and other symptoms.Each report includes comments from the Alere healthcare team
including adherence to medications and the physician’s prescribed treatment plan.
5.Medication Reports – Physician Medication Reports are designed to improve medication compliance.The report includes specific member prescription data along with medication therapy guidelines and comments.This report is available for all
programs to notify the physician when an enrolled participant is not on appropriate class medications,PBM data indicates that the member missed a medication refill,or for non compliance with physician prescribed prescription regimen.
Physicians with members on our program have found an increase in member adherence to treatment plans,overall improved me mber awareness of their illness,and a reduction in avoidable hospitalizations.
